- Please have copies of your pet's medical records, blood tests and/or x-ray films available for review at the first appointment.
- In addition, some forms will need to be completed at that time.
- As an added convenience, these forms can be pre-filled and printed out prior to our meeting.
- Please complete both the Client/Pet Info and the Medical History Forms.
